Scholarship Extension Petition

Your Jordan Credit Union scholarship funds are expiring. If you plan to use your scholarship funds for continuing education expenses during the next two (2) years and would like to request an extension – please complete this form.

By signing below, I understand and acknowledge that once my Extension Petition is approved, I will have two years to use my remaining Jordan Credit Union scholarship funds for approved secondary education expenses such as tuition, textbooks, and class fees. In the event I choose not to use the scholarship funds within the two year extension, scholarship balances of more than $20 will be forfeited in full and returned to JCU.
